Fees for the 2011-2012 school year are as follows:

To take advantage of the early discount, all forms and payment are due
back to the high school by May 15th. Please make checks
payable to CUSD 95. District 95 will no longer accept checks written
on a business account. Only personal checks, money orders,
cashier's checks, a credit card or cash will be accepted for payment.

Personal checks must include your
correct address and two viable phone numbers.

If you choose to pay by Visa or
MasterCard online through E-pay, please print a transaction receipt and
attach it to your registration forms. We will need this receipt as
proof of payment before we can process your registration materials. To access

Registration
Instructions:
Even if you take
advantage of the early discount and register by May 15, all students
must attend the August Walk-In Registration in order to take an
ID/Yearbook picture, and receive their schedules. Since this picture
will be used for both the ID and Yearbook, students should dress
accordingly. Picture order forms will be available. Parents do not
need to attend if fees are paid, registration paperwork and residency verification have
been completed. If students cannot attend any of the dates above, they
must wait until the first day of school to get their ID/Yearbook picture
taken, receive their ID and pick up their schedule. Parents will not be
able to pick up their student’s schedule.
The dates and times for our August
Walk-In Registration are:
Monday, August 8th
9:00 A.M. – 4:00 P.M. in the main gym
Tuesday, August 9th
11:00 A.M. – 7:00 P.M. in the main gym
Tuesday, August 16th
11:00 A.M. – 7:00 P.M. in the main office
How to use this guide:
Please click, print and fill out the forms below after carefully reading
all the information and return them along with your payment.
Similar to last year, there will be a
mandatory residency check for all students. You may provide proof of
residency during any of the Walk-In Registration dates, or you may also
provide proof of residency at the Administration Building located at 400
South Old Rand Road between the hours of 9:00 a.m. to 12:00 p.m. or 1:00
p.m. to 4:00 p.m. Monday through Thursday beginning July 1.
Remember all students must attend the August Walk-in Registration in
order to take an ID
picture and receive their schedules.

Residency
Statement Form - To be
completed annually - 1 per child.
- Sign and date the form with
your child's date of birth.
- Return these
documents to show proof of residency.

Registration Form
- To be completed annually - 1 per child

Instructions for identification of Race and Ethnicity
- Check that all
information is updated and accurate, making changes where
necessary.
- If there are no
changes, put initials in top right corner.
- A parent or
guardian signature is required at the bottom.
- Student's Full and Middle
name is required along with Ethnicity and Race.
- Complete Home Address -
P.O. Box numbers will not be accepted.
- Complete parent contact
information, including other parent or guardian and a current
email address.
- If your marital status has
changed, provide custody paperwork with documentation of date of
decree, custody arrangements including the
signature/notarized page.
- Complete Emergency Contact
Information - make sure all numbers are updated and accurate.

Band, Choir, Orchestra & Color Guard Participation Form -
To be completed annually - 1 per child
- To be completed
for those students enrolled in Band, Choir, Orchestra & those
participating in Color Guard.
- Fee is $50 per activity.
- Color Guard payment is due
after tryouts.

Authorization for Electronic Network Access Form
- To be completed annually - 1 per child
- Student section should be
completed and signed by your student.
- Parent section should be
completed and signed by parent or guardian.

Photo Publish/Release Form -
For
Freshman Only
- Print child's first and
last name on the top line of the form.
- Parent or Legal
Guardian should fill in the appropriate lines at the bottom.
- Note: If you choose to
not release your child's picture or production, please check the
appropriate line.

Health Office Emergency Information
Form
- To be completed annually - 1 per child
- Everyone must complete the
Health Office Emergency Form.

Freshman Physical Form
- All 9th graders are
required to have a physical form.

Senior Release Form
- To allow
senior students who have earned 18 or more credits to leave campus during specified open periods.

Senior Parking Form
- To allow
senior students with a valid driver's license and proof of
insurance who have earned 18 or more credits to park on campus.
